H&W Enterprises is seeking a Mental Health Skill Building Counselor to provide community-based support to adults working toward greater stability, independence, and daily functioning. This role supports clients in developing practical life skills, strengthening coping strategies, following individualized service plans, and connecting with appropriate community resources.

The ideal candidate is patient, reliable, organized, and comfortable working directly with clients in community-based settings. This position requires strong documentation habits, sound professional judgment, and a commitment to respectful, person-centered care.

Responsibilities

The Mental Health Skill Building Counselor may provide direct community-based services, assist clients with daily living and independent living skills, document progress toward service goals, coordinate with supervisors and providers, and help clients remain connected to needed resources and supports. Specific duties may vary by location, program requirements, credentialing standards, and client needs.

Provide direct mental health skill-building support in home and community settings;
Assist clients with daily living skills, independent living skills, and wellness routines;
Support clients in following individualized service plans and working toward identified goals;
Complete progress notes, service documentation, and required records accurately and on time;

Coordinate with supervisors, clinical staff, case managers, and community providers;
Help clients access appropriate community resources, appointments, and support services;
Maintain professional boundaries, confidentiality, and compliance with program requirements;
Participate in supervision, team meetings, required training, and quality improvement activities.

Skills/Experience

Bachelor’s degree in psychology, social work, counseling, human services, or a related field preferred;
Experience providing mental health, skill-building, case management, or community-based support services;
Strong documentation, time management, and organizational skills;
Ability to work professionally with adults experiencing behavioral health or daily living challenges;

Patient, dependable, and service-oriented approach;
Ability to work independently in home and community settings;
Strong communication skills with clients, families, providers, and internal teams;
Commitment to confidentiality, professional ethics, safety, and person-centered care.
